Notice:  The Florida Department of Education requires the following

statement:  These classes are not designed or intended to qualify the participants and graduates

for employment in hypnotism.  They are intended solely for certification examination preparation,

a vocational purposes, personal enrichment and enjoyment of the participants.I have enclosed the

required deposit for the examination preparation class indicated above.  A $300. non-refundable processing fee will be charged for participant cancellation within seven (7) days of class starting

date or may be held for deposit on the NEXT scheduled class.  Should Omni cancel a class for any

reason, all monies paid will be promptly refunded.  Balances are due and payable in full on or

before class starting date by credit card, cash, money order, bank certified or cashier check. No

personal/business checks accepted on final balance unless paid two weeks in advance of training

starting date.
